2CHealth Solutions is currently looking for a full-time Chronic Care Case Manager to join our growing organization. We specialize in care for patients with chronic diseases. Your primary duty would be to work remotely as a Case Manager. The Case Manager, who works as a Care Coordinator, is a trained healthcare professional who helps to address patients’ concerns and manage their chronic diseases.
Responsibilities, but not limited to:
- Monitoring and coordinating treatment and care plans.
- Educating patients about their disease condition.
- Connecting patients with healthcare providers, and evaluate their progress.
- Updating patient-related health information, insurance and financial information
- Ensuring all the immediate patient's concerns and requests are being addressed.
- Assessing and developing health care interventions with the care team
- Working in collaboration with other healthcare providers and care teams including behavioral health, disease care management, home health care, social work and community-based organization as needed.

License, Education and Experience:
- Certified Nursing assistant with appropriate experience
- Current Medical Assistant-Certified, Medical Healthcare Diploma.
- 1 Year of professional experience in the same vicinity.
