What You’ll Do
- Provide case management, rehabilitation, and support services in community-based settings
- Conduct mental health status assessments and assist with biopsychosocial assessments
- Develop, implement, and update individualized recovery and service plans
- Coordinate care and connect clients to community resources and supports
- Advocate for client needs including housing, medical, legal, and social services
- Assist clients with activities of daily living, transportation, and life skills development
- Provide education on mental health conditions, symptoms, and medications
- Collaborate with families, caregivers, and interdisciplinary team members
- Maintain accurate, timely, and compliant clinical documentation
- Ensure adherence to DCF guidelines and FACT program standards
What You Bring
- Bachelor’s degree in a behavioral science field
- Experience working with adults with severe and persistent mental illness (e.g., schizophrenia, bipolar disorder, personality disorders)
- Strong communication skills and ability to work with diverse populations
- Ability to make independent decisions, including responding to crises and emergencies
Preferred
- Targeted Case Management (TCM) Certification
- Experience in community-based behavioral health or FACT programs
- Familiarity with Microsoft Office and electronic health record (EHR) systems

Additional Requirements
- Successful completion of Level II Background, MVR, and Drug-Free Workplace screenings
- Ability to work independently and in community-based environments
- Completion of all required trainings within designated timeframes
- Strong attention to detail and compliance with documentation standards
Work Environment & Physical Requirements
- Combination of office-based and community-based work
- Regular standing, sitting, walking, and client interaction
- Occasional lifting up to 25 pounds
- Moderate noise level in professional and community settings
