Position Summary:
The Patient Navigator engages clients living with serious mental illness, and other vulnerable populations that may be at risk for HIV with linkage to health, mental health, social services, and substance use. The Patient Navigator uses a peer-led (PATH) approach to ensure adherence to treatment recommendations and other needs as identified by the program team. Specifically, the Patient Navigator implements navigation of services in the community using street outreach and mobile outreach strategies.
Reporting to the Program Coordinator, the Patient Navigator will have the following responsibilities:
Responsibilities:
· Engages in outreach activities to identify vulnerable populations who use substances and at risk for HIV as assigned by the Program Coordinator
· Supports Community Health Workers with HIV/HCV screenings and linkage to care.
· Provides patient navigation to assigned caseload and ensures treatment recommendations and service referrals are adhered to.
· Utilizes motivational interviewing to support with linkage to care and referral follow-through
· Utilizes unique engagement strategies to keep vulnerable individuals engaged in service navigation and treatment recommendations.
· Adheres to program protocols and procedures as directed by the Program Coordinator.
· Use local epidemiology data to guide teamwork and program activities.
· Ensures linkages to behavioral health and substance use are well coordinated and tracked.
· Support community engagement and surveillance activities based on assignments and community trends.
· Enters documentation as outlined by the organization’s standards
· Adhere to program reporting requirements and data collection
· Supports agency-wide initiatives and programs.
· Other duties as assigned.
Experience and Education:
● High School Diploma or GED preferred
● Minimum two (2) years of experience in customer service in a healthcare, non-profit, or social service setting.
● Experience working with vulnerable populations (substance use and individuals living with serious mental illness)
Knowledge, Skills & Abilities:
● Drivers license required
● Ability to work with individuals living with substance use, behavioral health, and unhoused challenges.
● Ability to demonstrate sensitivity, empathy, and understanding of vulnerable populations.
● Ability to use computer software systems
● Familiarity with community needs in DC and suburban counties
● Ability to perform 80% of field outreach and mobile outreach.
● Ability to foster a cooperative work environment and provide caring and non-judgmental client-centered services.
● Bilingual English/Spanish and/or other languages strongly preferred.
Pay: From $45,000.00 per year
