Colonial Club Senior Activity Center is located in Sun Prairie, WI. Our goal is to enhance the well being and independence of our older adults. The organization is a charitable, non-profit organization and serves all of NE Dane county. We have a family atomosphere within our organization.

```Duties```
- Provide case management services to older adults including information about services, programs and resources that assist them to experience daily life with dignity and security
- Conduct assessments and develop individualized service plans
- Coordinate and monitor the delivery of services to clients
- Collaborate with community resources to ensure clients' needs are met
- Advocate for clients' rights and access to services, maximizing their opportunities for self-sufficiency and choice
- Maintain accurate and up-to-date case records
attends educational training and conferences as required or relates to the position
```Skills```
- Strong background in social work or related field
- Knowledge of older adult programs and related resources
- Familiarity with Medicare, Section 8, and other public assistance programs a plus
- Excellent communication and interpersonal skills
- Strong organizational and time management abilities
Job Type: Full-time
Pay: $18.00 - $20.00 per hour
