This is an on-site role based in Grand Junction, Colorado.
We are open to qualified candidates who are willing to relocate to the Grand Junction / Western Slope area. Relocation assistance may be available for the right candidate.
$65,000 – $75,000 base salary, plus eligibility for a performance-based bonus of up to 10%.
Position Summary
The Operations Manager oversees coordinated delivery of personal care and home and community-based services for clients in the Grand Junction, CO market and surrounding Western Slope area. This role focuses on Colorado’s In-Home Support Services (IHSS) and Home and Community-Based Services (HCBS) programs under Health First Colorado (Colorado Medicaid), with primary emphasis on Homemaker (HMKR), Personal Care (PC/PCW), and Health Maintenance Activities (HMA) services. The position drives business growth, maximizes authorized service hours, ensures high client and caregiver satisfaction, maintains full compliance with state and HCPF requirements, and supports operational excellence in a fast-paced homecare environment. A significant portion of the caregiver workforce is family members; the remaining half requires proactive external recruiting and staffing to meet client needs.
Essential Functions
- Communicate effectively with clients, family caregivers, Authorized Representatives, and case managers to deliver high-quality, person-centered in-home services under IHSS and HCBS.
- Schedule and coordinate caregiver visits, ensuring strong utilization of authorized HMKR, PC, and HMA hours while proactively addressing staffing gaps or changes in client needs.
- Lead recruiting, hiring, onboarding, and retention of qualified caregivers (both family and non-family). Build and manage processes to consistently source and staff non-family caregivers for the approximately 50% of cases that require agency-recruited staff. Provide orientation on agency policies, documentation, Electronic Visit Verification (EVV), and program-specific requirements.
- Assess client needs, support development and updates to Plans of Care, and advocate effectively during assessments, reauthorizations, and renewals with case managers to secure appropriate service levels.
- Ensure full compliance with Health First Colorado, HCPF, IHSS, HCBS, and related guidelines, including timely documentation, EVV compliance, medical record gathering, and audit readiness.
- Manage staffing to minimize overtime, support payroll/billing processes, and resolve issues related to timesheets or authorizations.
- Build and maintain strong relationships with case management agencies, clients, families, and community stakeholders to support growth and retention.
- Travel within the designated Western Slope / Grand Junction territory as needed (up to 50-70%).
- Perform other duties as assigned.
Qualifications
- Required: Minimum 2-5 years of operations or management experience in Colorado Medicaid homecare, with direct knowledge of IHSS and HCBS programs. Proven success working with case managers on authorizations, care coordination, audits, and compliance for Homemaker (HMKR), Personal Care (PC/PCW), and Health Maintenance Activities (HMA) services.
- Strongly Preferred: Experience managing a mixed caregiver workforce that includes both family caregivers and agency-recruited staff; demonstrated success in recruiting and retaining non-family caregivers in a competitive market. Familiarity with Community First Choice (CFC), CDASS, or related self-directed options is a plus.
- Strong understanding of Colorado Medicaid terminology, including IHSS, HCBS, Homemaker services, Personal Care services, Health Maintenance Activities, nursing facility level of care, and person-centered planning principles.
- Proficient in EMR/EVV systems, Microsoft Office, and able to learn program-specific portals.
- Excellent interpersonal, organizational, and problem-solving skills; ability to work independently and collaboratively. Experience leading teams and building recruiting/staffing processes is highly valued.
- Reliable, punctual, customer-focused, and committed to HIPAA compliance and client confidentiality.
- Valid driver’s license and ability to travel locally.
