Position Overview
The Hospice Registered Nurse (RN) is responsible for providing skilled, compassionate nursing care to hospice patients in their homes and other care settings. The RN will assess patient needs, develop and implement individualized plans of care, manage symptoms, and coordinate services with physicians, patients, families, and the interdisciplinary team.
Responsibilities
- Perform comprehensive nursing assessments of hospice patients.
- Develop, implement, and update individualized plans of care based on patient needs.
- Provide skilled nursing care and symptom management in accordance with the patient's plan of care.
- Administer medications and treatments as ordered by the physician.
- Provide wound care, medication management, disease management, and other skilled nursing services as appropriate.
- Educate patients, families, and caregivers regarding disease progression, medications, symptom management, and end-of-life care.
- Communicate effectively with physicians, patients, families, caregivers, and members of the hospice interdisciplinary team.
- Serve as a patient advocate and provide compassionate support to patients and their families.
- Monitor changes in patient condition and communicate appropriately with the hospice team and physician.
- Participate in IDG meetings, case conferences, and care coordination.
- Complete accurate and timely clinical documentation using electronic healthcare documentation systems.
- Provide care in accordance with hospice policies, physician orders, and applicable Texas regulations.
- Perform skilled nursing procedures within the scope of the Texas Nurse Practice Act.
Requirements
- Current, unrestricted Texas RN license.
- Minimum of 1 year of clinical nursing experience.
- Previous hospice experience preferred.
- Strong clinical assessment and critical-thinking skills.
- Excellent communication and interpersonal skills.
- Strong organizational and time-management skills.
- Ability to work independently and effectively as part of an interdisciplinary team.
- Current Texas driver's license.
- Current CPR certification.
- Reliable transportation and ability to travel throughout the assigned service area.
- Professional appearance and demeanor.
- Ability to maintain accurate and timely clinical documentation.
- Experience with electronic healthcare documentation systems; Kinnser experience preferred.
Preferred Qualifications
- Previous hospice nursing experience.
- Experience with PICC lines.
- Experience with wound VACs.
- Experience with complex wound care.
- Experience with symptom management and end-of-life care.
