Physical Therapist and PRN Woodlake at New Hope About the Role Woodlake at New Hope is seeking a compassionate and driven Physical Therapist to join our premier clinical team. Key Responsibilities
- Comprehensive Assessment: Evaluate residents across all levels of care to develop individualized treatment plans that promote mobility, safety, and independence.
- Personalized Therapy: Deliver high-quality physical therapy interventions, utilizing our modern equipment and gym spaces to help residents achieve their personal goals.
- Collaborative Care: Partner closely with our interdisciplinary team—including nursing, occupational therapy, and speech-language pathology—to ensure a holistic approach to resident wellness.
- Documentation Excellence: Maintain accurate and timely clinical records, ensuring all treatments and progress are documented in compliance with state and federal regulations.
- Resident Advocacy: Serve as a supportive resource for residents and their families, providing education on mobility techniques and home safety to ensure a seamless transition between levels of care.
Qualifications
- Education: Graduate of an accredited Physical Therapy program.
- Licensure: Current and valid Physical Therapist license in the state of Minnesota.
- Experience: Experience in skilled nursing or long-term care preferred, but we welcome passionate clinicians who thrive in a fast-paced, multi-level care environment.
- Skills: Strong communication skills, a "resident-first" mindset, and the ability to adapt clinical techniques to meet the needs of both active seniors and those requiring intensive rehabilitation.
