Duties
- Assist clients with Activities of Daily Living (ADLs), including bathing, dressing, grooming, and mobility support
- Monitor patient health by checking vital signs and observing for any changes in condition
- Support medication administration and help maintain accurate care plans in compliance with HIPAA regulations
- Prepare nutritious meals tailored to individual dietary needs and assist with meal planning and cooking
- Use specialized equipment such as Hoyer lifts or wheelchairs to ensure safe transfers and mobility assistance
- Maintain a clean and safe environment through regular cleaning, laundry, and organization of personal spaces
- Provide emotional support and companionship to enhance mental well-being, especially for clients with dementia or memory care needs
- Document patient observations, care activities, and any incidents thoroughly to ensure continuity of care
Requirements
- Proven experience in home & community care, assisted living, nursing homes, or group homes preferred
- Knowledge of patient observation techniques, medication administration, and vital signs monitoring
- Certifications such as CPR, First Aid, and HIPAA compliance are highly desirable
- Experience working with individuals with developmental disabilities, autism, Alzheimer's disease or dementia care is a plus
- Ability to perform heavy lifting safely and operate patient transfer equipment like Hoyer lifts
- Strong communication skills to collaborate effectively with healthcare teams and family members
- Compassionate attitude with a commitment to providing respectful and dignified care
- Valid driver’s license and reliable transportation may be required for home visits
