Duties
- Assist clients with Activities of Daily Living (ADLs), including bathing, dressing, grooming, and mobility support using proper patient transfer methods such as Hoyer lifts when necessary
- Monitor patients’ vital signs and observe for any changes in health or behavior, documenting findings accurately in care plans
- Prepare nutritious meals tailored to individual dietary needs and assist with feeding as required
- Administer medications following prescribed instructions while adhering to HIPAA regulations to ensure confidentiality and privacy
- Support clients with medication administration, ensuring proper timing and dosage under supervision or as directed by healthcare professionals
- Provide companionship and emotional support to reduce feelings of isolation, especially for clients with dementia or neurodevelopmental disorders
- Maintain a clean, safe environment by performing light housekeeping tasks such as cleaning, laundry, and organizing personal spaces
- Assist with transportation for medical appointments or community outings, ensuring safety during transfers and travel
Requirements
- Proven experience working in home health, assisted living facilities, nursing homes, or group homes; experience with Alzheimer's care, memory care, or hospice is highly valued
- Knowledge of patient observation techniques, vital signs measurement, medication administration, and first aid/CPR certification
- Familiarity with manual handling equipment like Hoyer lifts and patient transfer methods to ensure safe mobility assistance
- Ability to work independently while following detailed care plans tailored to individual needs
- Strong communication skills for effective client interaction and collaboration with healthcare teams
- Experience supporting individuals with developmental disabilities or neurodevelopmental disorders is preferred
- Valid driver’s license and reliable transportation for client outings and appointments
- Compassionate attitude combined with physical stamina for heavy lifting and assisting with ADLs in a variety of home settings

Pay: $16.50 - $18.00 per hour
