We are seeking a qualified and compassionate Director of Admissions & Social Services to join our team at a prominent skilled nursing facility in Southern Maine. As a Director of Admissions & Social Services, you will be responsible for managing the inquiry, admission, and readmission processes from hospitals, communities, and other healthcare entities. Complete and maintain daily census efficiently and accurately
Receive referrals to evaluate and screen patients' conditions to admission criteria
Respond to inquiry calls from hospital discharge planners, families, and other referral sources
Verify insurance coverage, including Medicare, MaineCare, commercial, and private insurance
Manage the admission process by maintaining updated bed availability and community services information
Alert appropriate department heads and building staff of projected changes (e.g., admissions, bed changes, and discharges)
Responsible for data entry input for all admissions, readmissions, discharges, room changes, and other related changes
Ensure demographic information is current and accurately recorded
Conduct facility tours
Prepare and present ongoing admission-related information daily at team meetings
Maintain ALL admission inquiry data in the EMR system (Point Click Care)
Prepare admission reports by collecting, analyzing, and summarizing data and trends
Educate physicians, case managers, social workers, patients, and family members about the availability and benefits of skilled nursing services
Develop and maintain key relationships
