What We Offer:
Responsible for providing services to patients according to the physician’s plan of care. Responsible for coordinating home care services for therapy only patients, to include establishing and maintaining the patient’s plan of care. Responsible for the quality, customer service and financial management of assigned case load.
What We're Looking For:
- Education: 4 Year / Bachelors Degree, required. Graduate from an accredited PT program.

Experience: 2 years of clinical experience in Home Health, preferred.

Licensure/Certification: Appropriate state PT license, required.

BLS, required.

Valid Driver’s License, required.

Additional Skills (required): Some evening hours and weekends may be required.
