Responsibilities
- Assist patients with daily activities such as bathing, dressing, grooming, and mobility to promote independence and comfort.
- Monitor vital signs including blood pressure, pulse, temperature, and respiration rates accurately and report any abnormalities promptly.
- Support patient transfer methods using equipment like Hoyer lifts or manual handling techniques to ensure safety during movement.
- Maintain detailed medical documentation within Electronic Health Record (EHR) systems while adhering to HIPAA regulations to protect patient confidentiality.
- Provide specialized care for individuals with dementia, Alzheimer's disease, or other memory impairments by implementing tailored memory care strategies.
- Assist with medication reminders.
- Follow aseptic techniques during procedures such as catheter care or wound dressing changes to prevent infection and promote healing.
- Support patients with developmental disabilities or those requiring hospice or palliative care by delivering compassionate and respectful assistance.
- Use EMR/EHR systems proficiently for accurate documentation of patient care activities and medical records management.
Skills
- Proven experience working with geriatric patients in nursing homes or long-term care facilities.
- Familiar with EMR/EHR systems for medical documentation.
- Skilled in safe manual handling practices and heavy lifting techniques using equipment like Hoyer lifts.
- Strong caregiving communication skills with the ability to connect empathetically with patients of all ages—including pediatrics and those with disabilities.
- Understanding of HIPAA regulations and commitment to maintaining patient confidentiality at all times.
- Experience providing specialized dementia or Alzheimer’s care within memory or assisted living communities is preferred.
- Competence in infection control practices including aseptic technique during procedures involving infectious disease management.
