The Case Manager is responsible for a variety of duties, which could include responding to contacts seeking information regarding services and supports for individuals with intellectual and developmental disabilities, assembling information about the individual for eligibility determination, providing Case Management, being responsible for the assessment planning, coordination, and monitoring of services and supports provided to individuals with intellectual/developmental disabilities and their families as appropriate. Spanish bilingual preferred with a $1.00 increase in pay rate.
Requirements:
· Bachelor's degree required; or Five years of relevant experience in the field of LTSS, which includes Developmental Disabilities; or Some combination of education and *relevant experience appropriate to the requirements of the position.
· Case management and/or advocacy are strongly preferred.
· Knowledge of cognitive disabilities: cause, treatment, services for support, education, employment, recreation, therapy, and related medical terms/medications.
· Driving is a requirement for this position. Must be able to drive company vehicles.

- 2 full-time position available at $27.05-$29.75/hr. DOE/DOQ.
-Remote/Mobile position - must be able to attend in-person training and meetings in Fort Collins, CO and throughout Larimer County.
