Laurel Ridge Rehabilitation and Skilled Nursing Center, a proud subsidiary of Ascentria Care Alliance, is seeking a compassionate and dedicated Licensed Practical Nurse (LPN) to join our team in Jamaica Plain. What You Will Do (Key Responsibilities)
- Resident Care: Provide top-quality nursing services, conducting assessments of physical/emotional needs, monitoring for acute changes in condition, and managing care plans.
- Clinical Excellence: Obtain/document vital signs, assist with admissions and discharges, and ensure physician orders are implemented.
- Leadership: Provide oversight of Certified Nursing Assistants (CNAs) and licensed nurses as directed by the Director of Nursing.
- Collaboration: Facilitate physician rounds, flag concerns, and coordinate care with the IDT.
What We Need (Qualifications)
- License: Current and Valid MA LPN license is required.
- Experience: Previous experience in a Skilled Nursing Facility (SNF) or Geriatric setting is preferred.
- Technology: Experience with Electronic Medical Record documentation, specifically PointClickCare, is preferred.
