About Us:
Santana Hills Healthcare Center is a 167-bed facility with 90 skilled nursing beds and 77 sub-acute beds, delivering compassionate, patient-centered care for rapid recovery in a central San Jose location.
We are seeking a compassionate and motivated Occupational Therapist to join our healthcare team. The Occupational Therapist will evaluate, plan, and implement individualized treatment programs to help patients develop, recover, and improve the skills needed for daily living and working
Key Responsibilities

Assess patients’ physical, cognitive, and emotional abilities

Develop personalized treatment plans and therapy goals
-
Provide therapeutic interventions to improve independence and quality of life

Educate patients and families on therapy techniques and adaptive equipment
-
Maintain accurate patient records and progress notes
-
Collaborate with physicians, nurses, caregivers, and other healthcare professionals
-
Monitor patient progress and modify treatment plans as needed
-
Ensure compliance with healthcare regulations and safety standards
What We Are Looking For
-
Bachelor’s or Master’s degree in Occupational Therapy
-
Valid Occupational Therapist license/certification
-
Strong communication and interpersonal skills
-
Ability to work independently and as part of a multidisciplinary team
-
Experience in pediatric, geriatric, or rehabilitation settings is a plus
-
CPR certification preferred
