Responsibilities Provide medical assessments and develop care plans for residents in the skilled nursing facility Manage ongoing patient care, with focus on functional progression and overcoming barriers to improving function. Collaborate with nursing staff and interdisciplinary teams to ensure holistic patient care
- Conduct regular rounds to evaluate patient progress and adjust treatment plans as needed
- Educate residents and their families about health management and the role of post acute recovery
- Participate in quality improvement initiatives to enhance patient care practices
- Document medical histories, progress notes, and treatment plans accurately in the electronic health record
Requirements
- MD or DO degree from an accredited medical school
- Active medical license to practice in the state
- Board certification or eligibility in relevant specialty
- Experience in geriatrics or skilled nursing facilities preferred
- Strong communication and interpersonal skills to engage with patients and families
- Commitment to providing compassionate and patient-centered care
- Ability to work collaboratively in a team-oriented environment
