We are hiring Case Managers to join our team at ACT “Autism Care Therapy.".
Responsibilities:
- Provide case management services to children with autism and their families by assessing need (utilizing the IM+CANS), investigating available resources, and linking clients to identified supports in the areas of behavioral health, physical health, social, vocational, educational, housing, public income entitlements and other community-based resources
- Develop therapeutic rapport with clients and their families and meet with families on a scheduled basis to work towards individualized treatment goals
- Conduct multidisciplinary team meetings by collaborating with the child's ABA team and outside service providers to deliver comprehensive and personalized supports
- Provide oversight and guidance to Community Support personnel by developing programming, reviewing data, and making program modifications based on progress
- Adhere to all state and company policies and procedures
- Complete timely documentation of services provided in accordance with state and company standards
- Complete all assigned trainings and attend supervision meetings
- Other duties as assigned

- Strong interpersonal skills

- Experience with leading a team and advocating for clients
- Proficient in investigating resources and following up appropriately
Qualifications:
- Hold a bachelor's degree in human services field or have a high school diploma/GED and five years of documented clinical experience in mental health or human services
- 21 years or older
- Valid driver's license
- Must be able to pass a background check
- 1+ years of experience providing case management services
- 1+ years of experience working with individuals with developmental disabilities
- Certification in IM+CANS or ability to obtain certification within 30 days of hire
- Speaks Arabic, Spanish or Urdu
