Overview
Join a tech-enabled medical group delivering hospital-level acute care into patients' homes across Brooklyn and surrounding areas as a Nurse Practitioner or Physician Assistant, earning $176,800–$197,600 annually on a 3 x 12-hour schedule with four consistent days off every week. This is the highest-autonomy field provider role available, solo on-site clinical authority in the patient's home, no physician or paramedic backup, and a mission measurably reducing hospitalizations across New York's most vulnerable communities.
What You'll Do
- Perform independent hands-on assessments and interventions in the home: IV access, medication administration, wound care, and splinting
- Order and interpret point-of-care diagnostics including labs, EKGs, and POCUS
- Build individualized care plans with referring clinicians, honoring patient goals and palliative principles
- Coordinate safe care transitions and communicate across the interdisciplinary team
- Contribute to quality initiatives targeting readmission reduction and ED diversion
- Maintain accurate, timely documentation in compliance with regulatory requirements
- Transport and deploy field equipment across diverse home settings including stairs and non-clinical environments
Who You Are
- Active, unrestricted ARNP or PA-C license in New York
- Active, unrestricted driver's license
- Current BLS (ACLS preferred)
- Minimum 2 years or 3,600 hours in ED, inpatient medicine, or acute post-acute care
- Physically able to carry equipment up to 60 lbs and navigate stairs
- Comfortable with solo, unplanned care delivery in non-clinical home environments
- U.S. resident, eligible to work without sponsorship
- Available for evenings, weekends, and holiday coverage
- AGACNP or FNP certification accepted for NPs
Strongly Preferred
- AGACNP-BC, ENP-BC, or PA-C with NCCPA CAQ in Emergency Medicine or Hospital Medicine
- 5+ years of acute clinical practice
- EMS background including paramedic or critical care transport
- Hospital-at-Home, home-based primary care, or community paramedicine experience
Not a Fit For This Role
- AGPCNP-BC or ANP (same track, neither accepted)
- Any pediatric NP track (PNP-PC or PNP-AC)
- PMHNP-BC
- Aesthetic or cosmetic NP practice
- FNP-BC without 3,600+ documented hours in ED, inpatient, or acute post-acute settings
- PA-C primarily in dermatology, cosmetic, occupational health, or pediatrics
- Primary setting in outpatient primary care, urgent care, retail clinic, telehealth-only, school-based, occupational, or corporate health
- New graduates with less than 2 years post-licensure

- Some evening, weekend, and holiday coverage required
