Job Overview
We are seeking a compassionate and dedicated Certified Home Health Aide to provide exceptional in-home care to individuals with diverse needs. Duties
- Assist patients with Activities of Daily Living (ADLs) such as bathing, dressing, grooming, and mobility
- Monitor and record vital signs including blood pressure, pulse, temperature, and respiration
- Support medication administration following proper procedures and documentation standards
- Provide specialized care for individuals with dementia, Alzheimer's disease, or other memory impairments
- Operate Electronic Health Record (EHR) systems to document patient information accurately and securely
- Support patients with catheter care, wound management, and hospice care needs
- Use assistive devices such as Hoyer lifts to ensure safe transfers and mobility assistance
- Perform light housekeeping tasks including cleaning, laundry, meal preparation, and ensuring a safe environment
- Observe patient conditions closely, report changes promptly to healthcare professionals, and maintain HIPAA compliance for confidentiality
- Work with individuals with developmental disabilities or disabilities to promote independence and well-being
- Must speak Hindu and Punjabi.
Experience
- Prior experience in home health or home care settings is highly preferred
- Experience working in assisted living or nursing home environments is advantageous
- Knowledge of medical terminology and medical documentation practices is essential
- Demonstrated ability to provide patient-centered care for seniors or individuals with disabilities
- Hands-on experience with EHR or EMR systems for accurate record keeping
- Certification in CPR is required; additional certifications such as hospice care or dementia care are a plus
- Long-term care experience and familiarity with hospice or memory care settings will strengthen your application
