In this vital role, you will provide direct nursing care and support to individuals receiving services in a 23-hour observation and Residential Crisis Stabilization Unit (RCSU). This role supports individuals experiencing behavioral health crises by assisting with medical monitoring, medication administration, care coordination, and documentation to promote stabilization and continuity of care. coordinating with staff in regards to client appointments & medication pick up. This position primarily focuses on patient monitoring; paperwork documentation and medication administration.

Positions Available:
- Day Shift: Friday-Mondays
Key ResponsibilitiesClinical & Nursing Care
- Prepare, obtain, and document vital signs in accordance with facility protocol.
- Administer medications safely and accurately during scheduled medication passes.
- Perform and document blood glucose checks for diabetic clients during medication pass and as ordered.
- Monitor clients for changes in physical and mental health status and report concerns to the RN or provider
- immediately.
- Complete intake/admission assessments during shift, including individuals entering the 23-hour observation program.
- Ensure all required admission documentation is accurate, complete, and entered into the electronic health record (EHR).
- MUST be comfortable with transporting clients to and from urgent care
- MUST be comfortable arranging for med refills and pick up from the local pharmacy down the street
- MUST be comfortable in working independently between 2 buildings
- Complete discharge processes, including final assessments, medication reconciliation, and documentation.
- Provide ongoing monitoring of clients in crisis stabilization, including safety checks and behavioral observations.
- Assist in de-escalation of clients in crisis using trauma-informed care techniques.
- Maintain a safe and therapeutic environment at all times.
- Coordinate with pharmacy and clients on medication pick up
QualificationsRequired:
- Active, unencumbered Licensed Practical Nurse (LPN) license in the state of Virginia.
- Current CPR/BLS certification.
- Knowledge of behavioral health and crisis stabilization practices.
- Strong documentation and organizational skills.
Preferred:
- 1 year experience as an LPN
- Crisis stabilization or psychiatric settings
- Substance use treatment
- Familiarity with DMAS and DBHDS regulations
