SgtCare is a veteran-focused medical consulting, administrative support, and care-coordination company that works with U.S. military veterans navigating the VA disability process.
We help veterans organize their medical and service-related information, coordinate with appropriate licensed medical professionals, and connect with VA-accredited attorneys or agents when accredited representation is needed.
We are growing quickly and are looking for a highly organized, dependable Veteran Intake & Care Coordination Specialist to become a key part of our veteran-support operation.
This person will often be the first point of contact after a veteran enrolls with SGT Care and will be responsible for helping guide that veteran through the administrative and medical-coordination process from intake through completion.
This is a high-responsibility role. You will communicate directly with veterans, medical professionals, VA-accredited representatives, attorneys, and internal team members on a daily basis.
What You’ll Do Veteran Intake & Case Assessment
Conduct detailed intake calls with newly enrolled veterans using our established intake process.
You will gather information regarding:
- Military service history
- Branch, MOS/rating, deployments, and service dates
- Current VA disability rating
- Existing service-connected conditions
- Physical symptoms and medical history
- Mental-health concerns disclosed by the veteran
- Prior diagnoses and treatment
- Existing medical documentation
- Prior VA decisions and claim history
- Current goals and areas requiring additional professional review
Our intake process is designed to create a clear, organized picture of the veteran’s medical and service history so the appropriate professionals within our network can evaluate and assist them.
Coordinate Medical Evaluations
Based on the veteran’s intake information and established internal protocols, you will help coordinate appointments with the appropriate licensed medical professionals in our network.
This may include:
- Physicians performing comprehensive physical evaluations
- Medical providers evaluating physical conditions
- Licensed psychologists evaluating mental-health conditions such as PTSD when appropriate
- Other specialized medical professionals depending on the veteran’s circumstances
You will be responsible for:
- Scheduling appointments
- Providing providers with the necessary documentation
- Ensuring veterans understand their next steps
- Following up after appointments
- Collecting completed medical records and reports
- Keeping cases moving through the process
Coordinate With VA-Accredited Representatives
SgtCare works with VA-accredited attorneys and agents who may assist veterans when accredited representation is required.
You will help coordinate communication between the veteran and these professionals by:
- Sending appropriate case information
- Helping organize supporting records
- Coordinating introductions and appointments
- Tracking case status
- Following up on requested documentation
- Helping ensure that all parties have the information they need
You will serve as a liaison between the veteran, our internal team, the medical professionals, and accredited representatives.
Medical Documentation & Records Coordination
You will assist with organizing and preparing medical documentation for provider review.
Responsibilities may include:
- Organizing medical evidence
- Reviewing records for completeness
- Gathering missing records
- Preparing background information for medical providers
- Assisting providers administratively with documentation workflows
- Coordinating medical-opinion and nexus-letter workflows where appropriate
- Ensuring completed documents are properly stored and routed
Any medical opinion, diagnosis, or clinical conclusion must ultimately come from the appropriately licensed medical professional.
Athena / Medical Records Management
We use Athenahealth as part of our medical-records workflow.
You may be responsible for:
- Uploading records
- Retrieving documents
- Updating patient information
- Organizing files
- Monitoring provider documentation
- Ensuring records are properly maintained
- Coordinating information between providers and the veteran
Experience with Athenahealth or another electronic medical record system is a strong plus.
Veteran Communication & Case Management
You will maintain regular communication with veterans throughout their process.
That includes:
- Answering questions
- Explaining next steps
- Following up on missing documentation
- Confirming appointments
- Coordinating with providers
- Helping resolve scheduling issues
- Updating internal case notes
- Making sure cases do not stall
Many veterans we work with have complex medical histories and may be frustrated with the VA system.
You must be patient, professional, organized, and comfortable speaking with veterans who may be dealing with significant physical or mental-health challenges.

This is not a passive administrative role.
You will own cases, follow up aggressively, coordinate multiple professionals, and make sure veterans continue moving through the process.

HIPAA & Confidentiality
This position involves access to sensitive medical and personal information.
HIPAA training or experience working with protected health information is strongly preferred.
Location
This is primarily a remote position and we are open to qualified applicants nationwide.
However, we have a strong preference for candidates located in:
- Phoenix
- Scottsdale
- Tempe
- Mesa
- Chandler
- Gilbert
- Fountain Hills
- Other nearby Arizona communities
Our leadership team is based in the Phoenix / Scottsdale area, and we value the ability to occasionally meet in person for training, team meetings, or company activities.
Compensation
Starting at $50,000 with potential of$60,000 or more annually
Full-time position.
Schedule
Generally Monday through Friday during normal business hours.
Some flexibility may occasionally be required based on veteran appointments, provider schedules, and case needs.
