Job Overview
We are seeking a dedicated and highly skilled Wound Care Registered Nurse (RN) to provide expert wound management and patient care in Baldwin Park. The ideal candidate will possess extensive clinical experience, including wound assessment, treatment planning, and advanced nursing procedures. The Wound Care RN will collaborate closely with multidisciplinary teams to ensure optimal healing outcomes and uphold the highest standards of patient safety and comfort.
Duties
- Conduct thorough wound assessments, including documentation of wound size, type, and progress using electronic health record (EHR) systems such as Epic or Cerner.
- Develop individualized wound care plans incorporating advanced techniques such as suturing, debridement, and application of specialized dressings.
- Administer treatments including IV infusions, medication administration, and topical therapies like Botulinum toxin when indicated for specific conditions.
- Perform complex procedures such as catheterization, ostomy care, and sterile dressing changes following aseptic techniques.
- Educate patients and their families on wound management, infection prevention, and self-care strategies tailored to their needs.
- Collaborate with physicians, specialists in ophthalmology or cardiology, and other healthcare providers to coordinate comprehensive patient care.
- Maintain accurate medical documentation adhering to HIPAA regulations and hospital standards while utilizing EMR/EHR systems effectively.
- Participate in quality improvement initiatives aligned with NCQA standards and infection control protocols to enhance patient outcomes.
Requirements
- Valid RN license with current registration in the state of practice.
- Extensive clinical experience in wound care management within hospital or long-term care settings.
- Proficiency with EMR/EHR systems such as Epic, Cerner, or eClinicalWorks; familiarity with ICD-10/ICD-9 coding is preferred.
- Experience working with diverse populations including individuals with developmental disabilities, memory care needs, or in hospice & palliative medicine contexts.
- Strong knowledge of physiology, anatomy, and medical terminology relevant to acute and chronic wound treatment.
- Demonstrated skills in critical care environments such as ICU or Level I/II trauma centers; experience in emergency medicine or hospital medicine is advantageous.
- Ability to perform advanced procedures including laboratory specimen collection, phlebotomy, and sterile processing techniques.
- Excellent communication skills for patient education, triage assessments, and interdisciplinary collaboration.
