Job Summary:
We are seeking a Field Registered Nurse to join our Licensed Home Care Services Agency. The Field RN will be responsible for conducting patient visits, admissions, reassessments, supervisory visits, and care coordination in the home care setting.
This is a Fee-for-Service / Per Visit position, ideal for an RN who is comfortable traveling to patients’ homes throughout BROOKLYN.
Key Responsibilities:
Responsibilities may include additional tasks based on the needs of the department and organization.
- Conduct admissions, reassessments, and supervisory visits for home care patients in accordance with physician orders and agency policy.
- Coordinate and supervise Home Health Aides in the delivery of patient care.
- Initiate home care visits and reassess patient needs and progress on a regular basis.
- Initiate and update the plan of care under physician orders.
- Observe, assess, and document patient condition, progress, and any changes in care needs.
- Educate patients and caregivers regarding disease process, medications, plan of care, individualized treatment plans, and in-home care needs.
- Ensure accurate and timely clinical documentation in compliance with agency and regulatory requirements.
- Communicate with the clinical team regarding patient status, care concerns, and follow-up needs.
Qualifications:
Active New York State Registered Nurse license required.
Bilingual English/Spanish-Preffered
Prior home care experience preferred.
Strong clinical assessment, documentation, and communication skills.
Ability to travel throughout the Bronx and Manhattan.
