We are seeking a dedicated and proactive Admissions Coordinator/Discharge Planner to join our dynamic healthcare team. Duties
- Collaborate with medical staff to assess patient needs and determine appropriate admission or discharge plans.
- Coordinate with community resources, outpatient services, and post-acute care facilities to arrange timely discharges.
- Review patient medical records, diagnostic evaluations, and clinical documentation to support case management decisions.
- Facilitate communication between patients, families, healthcare providers, and social workers to ensure clear understanding of care plans.
- Manage electronic health records (EHR) systems accurately, maintaining HIPAA compliance and confidentiality at all times.
- Prioritize tasks effectively to handle multiple admissions and discharges efficiently while maintaining high-quality patient service.
- Assist in utilization management by evaluating the appropriateness of hospital stays and ensuring compliance with insurance requirements.
Requirements
- Proven experience working in hospital settings with exposure to acute care environments such as ICU, Emergency Medicine, or Level I/II Trauma Centers.
- Strong knowledge of medical terminology, physiology, and clinical case management principles.
- Experience with inpatient and outpatient discharge planning, including post-acute care coordination.
- Familiarity with electronic health records (EHR/EHR management) systems and healthcare data privacy regulations like HIPAA.
- Excellent communication skills for working with diverse patient populations, families, healthcare teams, and community resources.
- Ability to prioritize tasks efficiently in a fast-paced environment while maintaining attention to detail.
- Certification in Basic Life Support (BLS), CPR, or other relevant clinical certifications preferred; experience working with children or individuals with disabilities is a plus.
- Knowledge of behavioral health, mental health counseling, addiction counseling, or social work is advantageous for comprehensive patient assessments.
