Job Summary: As a Care Coordinator, you will play a vital role in ensuring the smooth and efficient delivery of home care services to our clients. You will be responsible for coordinating and managing care plans, assessing client needs, and collaborating with our dedicated team of caregivers and healthcare professionals. Your exceptional organizational and communication skills will be critical in maintaining high-quality care standards and fostering strong relationships with clients and their families.
Responsibilities:
- Conduct comprehensive assessments of client needs and develop individualized care plans
- Coordinate and schedule home care services based on client requirements, caregiver availability, and caregiver skillsets
- Match clients with suitable caregivers based on their preferences and needs
- Provide ongoing support and supervision to caregivers, ensuring adherence to care plans and maintaining high-quality standards
- Collaborate with healthcare professionals, including doctors, nurses, and therapists, to coordinate care and address any changes or concerns in client conditions
- Regularly communicate with clients and their families to ensure satisfaction with services and address any issues or concerns promptly
- Maintain accurate and up-to-date documentation of client information, care plans, and service delivery including placement strategies.
- Stay informed about industry trends, best practices, and regulatory requirements related to home care services
Qualifications:
- Prior experience (such as Nursing, Social Work, or Healthcare Administration) is preferred
- Proven experience in care coordination or a similar role within the healthcare or home care industry
- Strong knowledge of home care services, care plans, and assessment tools
- Excellent organizational and time management skills with the ability to prioritize tasks effectively
- Exceptional interpersonal and communication skills to build rapport with clients, caregivers, and healthcare professionals
- Ability to work collaboratively in a team environment while also being self-motivated and independent
- Proficiency in using technology and electronic health record systems
- Valid driver's license and access to reliable transportation
