ESSENTIAL FUNCTIONS:

Case Management and assessment of consumers assigned by Case Management Supervisor. This includes CCMEP.

May provide on-site case management services throughout Wayne County Schools

Maintains complete records on all consumers receiving services as assigned

Performs intake for consumers
-
Meets with consumers on a weekly basis to review progress on service plans and updates progress in the Consumer Database
-
Attends meetings and case staffings as required for each consumer
-
Initiates, maintains and documents service provided for consumers and updates in the consumer database
-
Coordinates daily consumer schedules
-
Prepares required reports on all consumers and trainees
-
Provides progress reports for the Case Management Supervisor on a weekly basis for everyone served.

Works directly with consumers and trainees in all phases of the rehabilitation process
-
Provides retention services to consumers
-
May function as a driver when needed

Performs other duties as assigned by the Case Management Supervisor and the Mission Services Director

QUALIFICATIONS:

Education: College degree in a Human Services-related field preferred, but not required (High school diploma or equivalent with relevant experience accepted)

Experience: 3 years in a related field preferred

Certification: CPR/First Aid (can be obtained upon hire)

Other: Valid driver’s license and insurability or ability to provide reliable transportation
