Job Summary:
Alpine Rehabilitation and Nursing Center is seeking a compassionate and skilled PRN Licensed Practical Nurse (LPN) to join our dedicated healthcare team. Responsibilities:
Provide direct patient care to our residents, including administering medications, performing vital signs, and assisting with activities of daily living
Develop and implement individualized care plans in collaboration with the patient, family, and healthcare team
Communicate effectively with patients and families, as well as other healthcare professionals, to ensure seamless care transitions
Participate in quality improvement initiatives to promote positive patient outcomes and optimize our care processes
Demonstrate a commitment to patient-centered care, dignity, and respect for all individuals
Requirements:
Current LPN licensure in the state of Ohio

Ability to maintain a professional, respectful, and empathetic demeanor in a fast-paced healthcare environment
