Duties
- Perform comprehensive patient assessments including vital signs, physical examinations, and health history documentation using Electronic Health Record (EHR) systems such as Epic or Cerner.
- Administer medications via injections, IV infusions, and other routes following strict aseptic techniques and clinical confidentiality policies.
- Manage complex care procedures including catheterization, tube feeding, ventilator management, and wound care in accordance with long-term care regulations and facility protocols.
- Conduct diagnostic evaluations such as specimen collection/processing, medical imaging coordination, and basic laboratory testing like phlebotomy.
- Provide specialized care for geriatric patients with dementia or Alzheimer’s disease in memory or hospice care settings; deliver pain management and acute pain interventions effectively.
- Collaborate with multidisciplinary teams on discharge planning, case management, and patient education to ensure seamless continuity of care across settings.
- Maintain accurate medical documentation including ICD coding (ICD-9/ICD-10), CPT coding for procedures, and detailed nursing notes while adhering to HIPAA standards.
Qualifications
- Valid LPN license with current state licensure required; previous travel nursing experience preferred.
- Demonstrated proficiency with EMR/EHR systems such as Epic, Cerner, or Athenahealth; experience with EMR/EHR management is highly desirable.
- Critical care experience including ICU or emergency room settings is strongly preferred; familiarity with Level I Trauma Center protocols is a plus.
- Extensive background in medical-surgical nursing, geriatrics, long-term care, hospice or palliative care; experience working with individuals with disabilities or developmental delays is advantageous.
- Knowledge of advanced nursing procedures such as IV insertion/infusion therapy, airway management, and infection control practices.
- Strong understanding of anatomy and physiology relevant to diverse patient populations; excellent patient assessment skills are essential.
- Ability to work independently in fast-paced environments while maintaining clinical excellence and compassionate patient interactions.
