Responsibilities
- Assist clients with Activities of Daily Living (ADLs), including bathing, dressing, grooming, toileting, and mobility support
- Monitor patients’ health status by observing vital signs, mental state, and overall condition; report changes promptly to healthcare professionals
- Prepare nutritious meals aligned with dietary restrictions and preferences; ensure proper food handling and safety protocols are followed
- Administer medications accurately according to care plans and document all medication administration activities in compliance with HIPAA regulations
- Support patients with mobility using equipment such as Hoyer lifts or other assistive devices; assist with transfers safely and effectively
- Maintain a clean, safe, and comfortable environment by performing light housekeeping tasks such as cleaning, laundry, and organizing living spaces
- Develop and implement personalized care plans in collaboration with healthcare teams; participate in ongoing training on dementia care, Alzheimer's care, hospice care, autism support, and other specialized areas
Qualifications
- Prior experience in home health, assisted living, nursing homes, or group home settings is highly preferred
- Knowledge of patient observation techniques and ability to monitor vital signs accurately
- Certified in CPR and First Aid; additional certifications such as HIPAA compliance or medication administration are advantageous
- Demonstrated ability to work compassionately with individuals with developmental disabilities, dementia, Alzheimer’s disease, or other memory care needs
- Strong communication skills to effectively collaborate with clients’ families and healthcare professionals
- Ability to perform heavy lifting when necessary; familiarity with equipment like Hoyer lifts is a plus
- Valid driver’s license and reliable transportation preferred for community-based visits
