Job Overview
We are seeking a dedicated and compassionate Licensed Practical Nurse (LPN) to join our healthcare team. The ideal candidate will possess a strong foundation in patient care, with expertise across various clinical settings. As an LPN, you will play a vital role in providing high-quality nursing services, supporting patient recovery, and ensuring compliance with healthcare regulations. Your responsibilities will include delivering personalized care, assisting with medical procedures, and maintaining accurate documentation to promote optimal health outcomes.
Duties
- Perform comprehensive patient assessments, including vital signs, physical examinations, and health history documentation using electronic health record (EHR) systems.
- Administer medications and injections accurately following physician orders while adhering to HIPAA and sterile technique standards.
- Assist with diagnostic evaluations such as specimen collection, processing, and basic medical imaging procedures.
- Support patients with activities of daily living, including tube feeding, catheter care, and mobility assistance using equipment like Hoyer lifts.
- Monitor patients’ conditions closely, recognizing signs of acute pain or deterioration requiring triage or escalation to higher levels of care.
- Collaborate with multidisciplinary teams in various settings such as long-term care facilities, assisted living communities, hospitals, and outpatient clinics.
- Maintain detailed medical records and ensure compliance with long-term care regulations and infection control policies.
- Provide education to patients and caregivers on medication management, health coaching, and discharge planning to promote ongoing wellness.
Qualifications
- Valid licensure as a Licensed Practical Nurse (LPN) in the respective state or jurisdiction.
- Proven experience working in diverse clinical environments including medical-surgical units, emergency rooms, ICU or trauma centers, hospice care, or skilled nursing facilities.
- Familiarity with EMR/EHR systems such as Epic, Cerner, Athenahealth, or eClinicalWorks for documentation purposes.
- Demonstrated proficiency in IV infusion techniques, catheterization, phlebotomy, and basic life support (BLS).
- Experience working with geriatric populations and individuals with developmental disabilities or dementia-related conditions such as Alzheimer’s disease or memory care needs.
- Knowledge of ICD coding (ICD-9/ICD-10), CPT coding for procedures, and familiarity with long-term care regulations.
- Strong communication skills for effective caregiver interaction and patient education across diverse populations.
- Ability to work efficiently under pressure in fast-paced settings like emergency medicine or Level I trauma centers while maintaining clinical confidentiality policies.
