VIGEO Home Health is seeking a Virginia-licensed Occupational Therapist for PRN home health visits in Sterling, Ashburn, and nearby eastern Loudoun County. This opportunity is designed for an OT who wants flexible supplemental work within a defined local territory.
This is a PRN W-2 position built around clinician availability and accepted assignments. Clinicians share the days, times, and local areas they can cover, and schedules may change as patient needs, physician orders, geography, and available assignments evolve. As our eastern Loudoun caseload grows, clinicians may choose additional assignments that fit their availability and preferred territory.

- Clinical and administrative support for care coordination, scheduling, documentation, and compliance.
- Interdisciplinary collaboration with nursing, physical therapy, physicians, and clinical leadership.
What you will do
- Complete occupational therapy evaluations and reassessments in patients' homes according to physician orders and the plan of care.
- Develop and update individualized plans of care addressing activities of daily living, upper-extremity function, cognition, safety, and functional independence.
- Provide skilled interventions, patient and caregiver education, home programs, adaptive-equipment recommendations, and home-safety training.
- Coordinate changes in condition and treatment recommendations with physicians and the interdisciplinary care team.
- Complete timely clinical documentation in the agency's electronic medical record, including required reassessment, recertification, and discharge documentation.
- Supervise Occupational Therapy Assistants and Home Health Aides when assigned and permitted by the plan of care and applicable requirements.
- Participate in targeted orientation, clinical training, and ongoing competency validations.
Required qualifications
- Current Virginia Occupational Therapist license in good standing.
- Degree in Occupational Therapy from an accredited program.
- Current CPR certification.
- Valid driver's license, reliable transportation, and automobile insurance meeting applicable agency and state requirements.
- Ability to travel to patient homes in the Sterling/Ashburn service area.
- Ability to work independently in the field while communicating promptly with patients, caregivers, physicians, and the care team.
- Ability to complete required onboarding and competency requirements before independently providing patient care.
Preferred qualifications
- Home health or Medicare-certified home health experience.
- Experience with geriatric, orthopedic, neurological, post-hospitalization, cognitive, or home-safety needs.
- Experience using WellSky/Kinnser or a comparable home health electronic medical record.
- Highly adaptable to modern clinical operations platforms, custom mobile workflows, and digital documentation tools.
- NBCOT certification.
