Job Summary
We are seeking a dedicated and energetic Rehabilitation Specialist for our Youth Program to join our dynamic team. Responsibilities
- Conduct thorough patient assessments and intake interviews to determine individualized treatment plans.
- Implement behavior management strategies and crisis intervention techniques to ensure safety and promote positive behavior change.
- Provide direct support and counseling for children, adolescents, adults, and families dealing with mental health issues, developmental disabilities, or substance use challenges.
- Collaborate with multidisciplinary teams on patient care plans, social work initiatives, special education programs, and hospital or community-based services.
Qualifications
- Active LMSW license required
- Experience working with mental health and community-based services preferred
- Strong documentation and case management skills
- Ability to collaborate with PRP staff, clients, therapists, and community providers
- Strong background in behavioral health counseling, mental health therapy, or clinical social work.
- Familiarity with crisis management protocols and crisis intervention strategies in diverse settings.
Pay: $35.00 per hour
