Job Overview
We are seeking a compassionate, energetic Wellness Nurse to join our Assisted Living Team. Responsibilities
- Conduct thorough patient assessments, including vital signs, physical examinations, and health histories to evaluate current health status.
- Administer medications and treatments accurately, per Physician orders following all safety protocols and documentation standards.
- Provide Initial and quarterly assessments, develop and revise resident care plans per LDH guidelines.
- Provide assisted living and specialized Alzheimer's/dementia care, and support to residents with cognitive impairments in compliance with assisted and memory care regulations.
- Collaborate with physicians and other healthcare professionals on assessments, evaluations and medication management
- Maintain detailed medical documentation while ensuring compliance with HIPAA confidentiality policies.
- Maintain Medication Administration Records.
Experience
- Proven experience working as a Nurse in diverse settings such as long-term care facilities, skilled nursing facilities, assisted living communities, or hospitals.
- Familiarity with dementia care, hospice care, home health care or memory support programs, enhances your ability to meet resident needs compassionately.
Pay: $27.00 - $30.00 per hour
