Duties
- Perform thorough patient assessments, including vital signs, physical examinations, and health history documentation using electronic health record (EHR) systems such as Epic and Cerner.
- Administer medications and injections accurately following prescribed protocols, including IV infusions and medication management for complex cases like acute pain and hospice care.
- Assist with specialized procedures such as catheterization, tube feeding, venipuncture, and sterile dressing changes while maintaining aseptic technique.
- Monitor patients’ conditions closely, utilizing critical care experience to identify changes requiring urgent intervention or triage decisions.
- Collaborate with interdisciplinary teams to develop and update individualized care plans for patients with diverse needs such as dementia, Alzheimer’s disease, or developmental disabilities.
- Document all patient interactions meticulously in medical records, ensuring compliance with HIPAA regulations and ICD coding standards like ICD-10 and ICD-9.
- Support discharge planning and post-acute care coordination to ensure seamless transitions for patients returning home or to assisted living facilities.
Requirements
- Valid LVN license in the state of practice with current certification.
- Proven experience working in various healthcare environments such as hospitals (including Level I Trauma Centers), nursing homes, assisted living facilities, or urgent care clinics.
- Strong knowledge of physiology, anatomy, medical terminology, and diagnostic evaluation techniques.
- Hands-on experience with EMR/EHR systems such as Epic, Cerner, eClinicalWorks, or Athenahealth.
- Competence in performing patient assessments, vital signs measurement, medication administration—including injections—and specimen collection/processing.
- Ability to work effectively with individuals with disabilities or complex health conditions like dementia or infectious diseases.
- Basic life support (BLS) certification required; additional certifications such as ACLS or PALS are a plus.
- Excellent communication skills combined with a compassionate approach to patient care in fast-paced settings like emergency rooms or ICU units.
