What You’ll Do
As an RN Case Manager, you’ll be the central point of care for your patients, ensuring they receive comfort, dignity, and expert support. Your days may include:
- Serving as the primary nurse for an assigned group of hospice patients
- Providing symptom management, patient education, and emotional support
- Developing, implementing, and updating individualized care plans with the IDG
- Communicating clearly with patients, families, and the care team so everyone feels supported and informed
- Documenting care with timeliness and accuracy, reflecting both quality and compliance
- Living out AlēvCare’s mission and values in every interaction
What We’re Looking For
- Current RN license in Texas (required)
- 2+ years of hospice RN experience preferred (we’ll also consider strong home health experience with a heart for hospice)
- Strong clinical judgment paired with emotional intelligence
- Valid Texas driver’s license and auto insurance
- Physical ability to lift/push/pull moderate loads (with occasional heavier lifting)
