About the Opportunity:

Schedule: Monday to Friday

Hours: 9am to 5pm

Setting: Supportive housing program
Responsibilities:

Providing case management services
-
Developing and monitoring support plans
-
Assisting with problem-solving and crisis intervention
-
Accompanying consumers to appointments

Acting as an advocate for consumers

Performing other duties, as needed
Qualifications:
-
1+ year of related work experience
-
High School Diploma / GED
-
Solid assessment and documentation skills
-
Patient oriented
-
Great interpersonal and communication skills
-
Strong attention to detail
-
Highly organized
Desired Qualifications:
-
3 years of experience with Behavioral Health populations

Bachelor's Degree

Experience with Scattering Site settings

Solid understanding of Supportive Housing dynamics
