CollaborateHealth partners with hospitals to reduce preventable readmissions under the Hospital Readmissions Reduction Program (HRRP). Through our proprietary technology platform and structured clinical workflows, we support patients during the critical transition from hospital to home by providing proactive clinical outreach, medication reconciliation, patient education, follow-up care coordination, and early intervention to help improve outcomes and prevent avoidable readmissions.
Position Summary
We are seeking a compassionate, organized, and proactive Nurse Practitioner who enjoys building rapport with patients over the phone, is comfortable working independently in a remote environment, and is passionate about improving patient outcomes through education, early intervention, and preventive care to join our remote clinical team on a part-time Mon - Fri, 9 AM to 1 PM ET contract basis. The Nurse Practitioner will conduct structured phone interviews with patients following hospital discharge, document all patient interactions within the CollaborateHealth platform, identify and manage patient escalations, and collaborate with the interdisciplinary care team to support high-quality transitional care. This role also participates in quality assurance, clinical auditing, and continuous process improvement initiatives.
Essential Responsibilities
· Conduct scheduled patient interview phone calls using established clinical workflows and protocols.
· Perform comprehensive post-discharge assessments and identify clinical or social barriers to recovery.
· Document all patient encounters accurately and in a timely manner within the CollaborateHealth platform.
· Identify, triage, and manage patient escalations, ensuring timely coordination with physician offices, specialists, home health agencies, pharmacies, and other members of the patient's care team as appropriate.
· Educate patients and caregivers regarding medications, follow-up care, symptom monitoring, and available resources.
· Communicate clinically significant findings to the appropriate care team members.
· Participate in quality assurance activities, chart reviews, and clinical documentation audits.
· Assist with workflow improvement initiatives and maintain compliance with company policies and clinical standards.
· Attend team meetings, training sessions, and case review discussions as scheduled.
· Perform additional clinical and administrative duties as assigned.
· Maintain productivity and quality metrics established by CollaborateHealth.
· Maintain patient confidentiality and HIPAA compliance in all patient communications and documentation.
Qualifications
· Current, unrestricted Nurse Practitioner license.
· Minimum of 3 years of Nurse Practitioner clinical experience,
· Excellent telephone communication and patient interviewing skills.
· Strong clinical judgment and ability to recognize and appropriately escalate patient concerns.
· Experience documenting in electronic health record or care management platforms.
· Strong organizational skills with attention to detail.
· Ability to work independently in a remote environment while collaborating effectively with a multidisciplinary team.
· Ability to travel as needed for onsite client meetings and check-ins.
· Preferred – Fluent in Spanish.
