As a Patient Services Coordinator, you'll play a vital role in keeping our clinical team organized and our patients cared for by:

Coordinating and scheduling home health visits for clinicians

Prioritizing new patient admissions to ensure timely starts of care

Communicating with nurses, therapists, patients, physicians, and referral partners

Managing scheduling workflows within our electronic medical record system

Providing outstanding customer service to patients and healthcare professionals

Assisting with medical records functions through cross-training as a Medical Records Specialist

Supporting quality improvement initiatives and agency operations

Helping ensure every patient receives the highest level of coordinated care
Clinical (LVN/LPN) candidates may also have the opportunity to perform patient visits as assigned.

High school diploma or equivalent required
-
LVN/LPN license preferred but not required

One year of home health or hospice experience preferred

Strong organizational, multitasking, and communication skills

Experience working in a fast-paced healthcare environment is a plus

Reliable transportation and valid driver's license

CPR certification required only if performing patient visits
