The Hospice Physician collaborates with the Hospice Medical Director to provide comprehensive hospice medical care in accordance with federal regulations and each patient’s established plan of care. This role reports directly to the Hospice Medical Director and plays a key part in ensuring high-quality, patient-centered end-of-life care.
Host and facilitate Interdisciplinary Team (IDT) meetings every two weeks in compliance with Medicare regulations
Assist the IDT in developing and implementing individualized patient plans of care in coordination with the attending physician
Review patient eligibility for hospice services; certify initial admissions and recertifications based on prognostic indicators
Serve as physician designee in the absence of the Hospice Medical Director
Provide medical management in consultation with attending physicians, consistent with patient needs and plans of care
Communicate with attending physicians and healthcare providers regarding hospice policies, procedures, and standards
Review medication profiles for drug interactions and appropriate hospice coverage
Provide medical coverage and clinical support to hospice staff during after-hours or when the attending physician or Medical Director is unavailable
Maintain ongoing communication with healthcare professionals regarding patient plans of care
Act as a liaison to community physicians; provide consultation and education to colleagues, referral sources, patients, and families regarding hospice admission criteria
Complete administrative duties including electronic or faxed prescription approvals, regulatory documentation, and care coordination communication
Advocate for patients, ensuring medical and psychosocial needs are met
Demonstrate current knowledge of pain and symptom management protocols
