Schedule:
- Full-time, Monday through Friday
- On-call likely as part of assignment
What you'll be doing:
- Deliver in-home skilled nursing interventions and compassionate care
- Perform thorough patient assessments and complete OASIS documentation with precision and timeliness
- Coordinate with physicians and interdisciplinary teams to create and implement individualized care plans
- Provide wound care, medication management, disease monitoring, and patient/family education
- Communicate consistently with the clinical team regarding patient updates and changes
Requirements:
- Active Registered Nurse (RN) license
- At least one year of recent home health nursing experience
- Strong OASIS experience required
- HomeCare HomeBase proficiency is strongly preferred
- Ability to maintain a consistent full-time schedule including daily patient visits
- Valid driver's license and reliable transportation
