Youable Emotional Health is committed to providing responsive, client-centered services that promote positive outcomes for children, families, and communities. We are seeking a compassionate, organized, and collaborative Case Management Intake Clinician to serve as the entry point for Case Management services and help ensure equitable access to care.
This position plays a critical role in coordinating referrals, managing waitlists, completing diagnostic assessments, and connecting clients with the resources they need in a timely and supportive manner.
Responsibilities

Screening external diagnostic assessments for Case Management eligibility.

Managing the CMDA waitlist including outreach to referrals to pre-register clients and schedule DAs, assigning DAs to themselves and CMDA-trained clinical providers as necessary.

Working with referrals to expedite the intake paperwork process.

Maintaining ongoing and timely documentation of outreach and services.

Managing the Case Management waitlist, including identifying clients needing insurance navigation, making assignments in collaboration with Case Management Supervisors, and offering triage resources to those waiting for services.

Coordinating with supervisors, Senior Case Managers, Client Experience Team, and billing to ensure up-to-date data tracking and marketing information.

Completing Diagnostic Assessments, including clinic-based, in-home, and DC 0-5 Assessments to determine Case Management eligibility (estimated nine to 15 per month; range based on travel and interactive complexity).

Potential of holding a small caseload of therapy clients based on agency need and clinical development of the team member.

Attending meetings to discuss intake trends with program stakeholders.

Other duties as assigned based on program and agency needs.
Qualifications
Education and Experience:

Master of Social Work (MSW) degree.

LGSW licensure (required), LICSW licensure (preferred).

Employed as a Case Manager for one or more years at Youable.
Skills and Abilities

Organizational skills to complete documentation.

Strong relationship-building skills and a desire to serve diverse populations.

Willingness to travel for work, with a working vehicle, car insurance, and driver's license.

Willingness to complete 0-5 training within six months of hire.

Willingness to work some evenings to meet client scheduling needs.
