Job Summary
My Home Hospice is seeking an experienced, compassionate, and motivated Hospice Clinical Liaison RN / Admissions Nurse to join our growing team. This hybrid position combines business development, community outreach, clinical assessment, and hospice admissions to help patients and families access compassionate end-of-life care.
The ideal candidate is passionate about building trusted relationships with physicians, hospitals, case managers, skilled nursing facilities, assisted living communities, and other healthcare providers while utilizing strong clinical expertise to assess hospice eligibility, educate patients and families, and coordinate seamless admissions into hospice services.
This role is ideal for an RN who enjoys both community engagement and direct patient care while making a meaningful difference in the lives of others.
Business Development & Community Outreach
- Develop and maintain strong referral relationships with physicians, hospitals, discharge planners, case managers, skilled nursing facilities, assisted living communities, home health agencies, and other healthcare partners.
- Represent My Home Hospice through professional networking, educational presentations, healthcare events, and outreach activities.
- Educate referral sources, patients, and families regarding hospice philosophy, eligibility criteria, Medicare Hospice Benefits, and available services.
- Serve as a trusted clinical resource by responding promptly to referral inquiries and facilitating timely patient evaluations.
- Maintain exceptional customer service and ongoing communication with referral partners.
- Collaborate with leadership to support ethical census growth through relationship-building and service excellence.
- Admissions & Clinical Responsibilities
- Respond promptly to hospice referrals and perform comprehensive hospice eligibility evaluations.
- Conduct hospice admission visits in homes, hospitals, skilled nursing facilities, assisted living facilities, and other care settings.
- Perform comprehensive nursing assessments and accurately document clinical findings within the EMR.
- Review hospice philosophy, patient rights, Medicare Hospice Benefits, consent forms, financial information, and admission paperwork.
- Obtain physician certification and admission orders while coordinating required documentation.
- Initiate the individualized plan of care in collaboration with the Hospice Medical Director and Interdisciplinary Group (IDG).
- Coordinate durable medical equipment (DME), medications, supplies, and ancillary services.
- Educate patients and caregivers regarding disease progression, symptom management, medications, emergency preparedness, and hospice services.
- Ensure a smooth transition into hospice care through effective interdisciplinary communication.
Compliance & Documentation
- Maintain compliance with Medicare Conditions of Participation (CoPs), CHAP accreditation standards, state regulations, and agency policies.
- Complete accurate, thorough, and timely clinical documentation in the EMR.
- Participate in interdisciplinary team meetings and quality improvement initiatives.
- Maintain patient confidentiality and uphold the highest standards of ethical nursing practice.
Qualifications – Required
- Current unrestricted Registered Nurse (RN) license in the State of Texas.
- Minimum of two (2) years of clinical nursing experience.
- Excellent communication, presentation, and relationship-building skills.
- Strong clinical assessment, critical thinking, and organizational abilities.
- Ability to work independently, prioritize responsibilities, and manage a flexible schedule.
- Valid Texas driver's license, reliable transportation, and current automobile insurance.
Preferred Qualifications
- Hospice, palliative care, oncology, home health, or case management experience.
- Previous hospice admissions experience.
- Business development, physician liaison, or healthcare marketing experience.
- Working knowledge of Medicare Hospice Conditions of Participation and hospice eligibility guidelines.

Pay: $70,937.88 - $85,430.57 per year
