Job Summary

Duties
- Conduct thorough patient assessments to determine care needs, including vital signs, physiology evaluation, and symptom management.
- Develop and implement individualized care plans in collaboration with physicians, social workers, and other healthcare team members.
- Manage medication administration, symptom control, and comfort measures to enhance patient quality of life.
- Provide education and emotional support to patients and their families about disease progression, treatment options, and end-of-life planning.
- Coordinate discharge planning and post-acute care services to ensure smooth transitions between settings.
- Maintain accurate medical records using Electronic Health Records (EHR) systems while adhering to HIPAA regulations for patient privacy.
- Participate in interdisciplinary team meetings to optimize patient service delivery and clinical case management strategies.
Requirements
- Valid nursing license with current registration in the state of practice.
- Critical care experience preferred, including ICU or emergency room backgrounds; Level I Trauma Center experience is a plus.
- Demonstrated knowledge of hospital medicine, post-acute care, and home health services.
- Experience with medical-surgical nursing, infectious disease care, geriatrics, behavioral health, or pediatrics is highly desirable.
- Proficiency in EMR/EHR management systems and medical terminology for accurate documentation.
- Strong understanding of health regulation policies such as HIPAA and utilization management protocols.
- Excellent patient service skills with the ability to communicate effectively with diverse populations and collaborate with healthcare professionals.
- Ability to manage multiple cases efficiently while maintaining a compassionate approach to patient care.
- In-depth knowledge of physiology, diagnostic evaluation procedures, discharge planning processes, and case management techniques.
