Job Overview
We are seeking a motivated Patient Care Coordinator who has provided care to patients with Hepatits, HIV, STD and worked in the public health space. Must be willing to travel throughout palm beach and other counties in South Florida as needed to join and strengthen our outpatient medical services team. Must be comfortable working in secured medical areas in correctional facilities occassionally. This role is ideal for organized, adaptable professionals who work well in a paired, team-based environment and are passionate about delivering high-quality patient care across community clinics and secure correctional facilities in Palm Beach County and at times in Lee County in Florida.
This position requires strong phlebotomy skills, flexibility, professionalism, and the ability to work collaboratively in dynamic care settings.
Duties
- Perform basic health screenings and blood and urine specimen collection (phlebotomy experience required)
- Assist with patient intake, scheduling, follow-ups, and EMR/EHR documentation
- Work as part of a two-person care coordination team at community sites and correctional facilities
- Collaborate with clinical staff to ensure continuity and quality of care
- Travel locally and regionally within Florida to support outpatient and community-based services
- Maintain professionalism and patient confidentiality in secure environments
Qualifications
- 1+ year of experience in phlebotomy, medical assisting, or a related clinical field
- Phlebotomy/specimen collection experience required
- Knowledge of HIV/STD prevention, counseling, or risk assessment preferred
- Strong organizational, communication, and documentation skills
- Comfortable working with diverse populations and in secure correctional facilities
- Ability and willingness to travel throughout Florida
- Occasional Saturday availability if needed
- Comfortable using EMR/EHR systems (or willing to learn)
- Demonstrated compassion, professionalism, and excellent interpersonal skills
Pay: $21.00 - $23.00 per hour
