Bayside Wellness Hospice is seeking a full-time Nurse Practitioner to join our growing team.
We're a locally owned, physician-led hospice that believes great patient care starts with good people. We are looking for someone who enjoys caring for older adults, values autonomy, communicates well, and wants to be part of a small organization where their ideas and work genuinely matter.
This is not a traditional hospice position. While hospice care will be an important part of your practice, you'll also help care for medically complex older adults in nursing facilities and assisted living settings. As our organization grows, so will the opportunities within this role.
Responsibilities
- Perform hospice face-to-face recertification visits.
- Complete hospice admission evaluations as needed.
- Evaluate patients experiencing changes in condition and assist with symptom management.
- Provide follow-up care for medically complex geriatric patients.
- Assist with physician vacation coverage and shared call responsibilities.
- Communicate with patients, families, nursing staff, pharmacies, consultants, and community providers.
- Participate in quality improvement initiatives and help develop new clinical programs as the organization grows.
Qualifications
- Licensed Nurse Practitioner in Michigan (or eligible for licensure).
- DEA registration (or ability to obtain).
- Excellent communication and organizational skills.
- Comfortable working independently while collaborating with physicians and interdisciplinary teams.
- Experience in hospice, geriatrics, family medicine, internal medicine, post-acute care, or palliative care is preferred but not required.
Compensation
- Starting salary of $93,000 - $95,000
- IRS mileage reimbursement
