Responsibilities
- Assist clients with activities of daily living (ADLs), including bathing, dressing, grooming, and personal hygiene
- Support with medication administration and monitor vital signs to ensure health stability
- Provide patient observation and monitor changes in condition, reporting any concerns promptly
- Prepare nutritious meals tailored to dietary needs and assist with meal planning and cooking
- Use proper manual handling techniques and patient transfer methods, including Hoyer lifts and other equipment
- Maintain accurate care plans, document patient progress, and communicate effectively with healthcare teams
- Perform light housekeeping tasks such as cleaning, laundry, and maintaining a safe living environment
- Offer companionship and emotional support to promote mental well-being
- Assist with transportation for medical appointments or community outings in a safe manner
- Follow HIPAA guidelines to ensure client confidentiality and privacy at all times
Experience
- Prior experience working in group homes, assisted living facilities, nursing homes, or home health environments preferred
- Demonstrated ability to provide patient personal care assistance and support individuals with disabilities or neurodevelopmental disorders
- Knowledge of dementia care, Alzheimer’s care, memory care, hospice care, and long-term care practices
- Familiarity with patient monitoring techniques including vital signs measurement and medication administration
- Experience working with individuals with autism or developmental disabilities is highly valued
- Certification in CPR and First Aid is required; additional certifications such as Hoyer lift training or manual handling are advantageous
- Strong communication skills for effective client interaction and collaboration with healthcare teams
- Ability to perform heavy lifting safely and assist with patient transfers using appropriate methods
