Position Title: Independent Contractor Registered Nurse (RN)/LPN
Department: Adult Foster Care
Reports To: AFC RN Manager
Location: Metro West Area

Whereas Agency operates as Victory Human Services at the address set forth above, and Agency desires to enter an agreement with Contractor as a Registered Nurse in the State of Massachusetts to provide nursing services according to Program Regulations
(130 CMR 408.415) (B):
- completing a nursing assessment and MDSs as required.
- coordinating all other applicable clinical assessments.
- developing the members’ AFC plan of care.
- conducting on-site visits with each member at the qualified setting:
(a) for Level I, quarterly (alternating with the bimonthly visit by the Care Manager.
(b) for Level II, bimonthly, or more often as the member’s condition warrants.
- completing a nursing progress note for each on-site visit or encounter and
upon meaningful change.
- monitoring each member’s health status and documenting those findings in
the member’s medical record for each on-site visit or encounter, or more often as the
member’s condition warrants.
- educating the member about hygiene and health concerns.
- reporting changes in the member’s condition to the member’s physician.
- coordinating the implementation of physician's orders with the members,
AFC Caregiver, and AFC Provider personnel.
- and evaluating, supporting, and training the AFC Caregiver.
1. Employment of Contractor as an Independent Consultant.
By this Agreement, Agency employs Contractor as a RN Nurse Independent
Consultant to provide nursing services for agency’s AFC members as set forth in RN
Nurse services above.
Contractor will have flexible work hours on a weekly basis and will work at his/her
own schedule to complete the assigned task.
Contractors will be required to attend a monthly RN Nurse and Multi-disciplinary meeting/training scheduled by AFC.
Contract will be required to participate in an annual Performance Review that
will allow for feedback from the consultant, members serviced, Caregivers, and
immediate Supervisor.
2. Registered Nurse Payment.
- Agency shall pay the Nurse $250 per initial Intake Assessment of a
member.
- Agency shall pay the Nurse $ 55 per Visit of a member within the
agreed radius from Victory Human Services’ (Brockton and Boston)
administrative offices.
- Agency shall pay the Nurse $55.00 for six-month summaries.
- Agency shall pay $35.00 for administrative work, which is therein defined
as
A. Training
B. Review meetings with the supervisor.
C. MDT meetings
D. Any meetings deemed official by the agency.
All payments shall be due and payable for five business days after submission of a bi weekly invoice and bi-weekly required documentation.
Qualifications
Required
- Bachelor's Degree, preferably in nursing
- Registered Nurse or LPN
- The registered nurse must be fully licensed by the Massachusetts Board of Registration in Nursing
- The registered nurse must have at least two years’ recent experience in the direct care of the elderly or disabled adults. Bottom of Form
- Have experience working in Human Services
- Exceptional communication, executive presence, and relationship-building capabilities.

- Must be skilled in the use of MS Word

- Commitment to improve care in underserved populations
- Collaborative work style; relationship building; entails meeting directly with
members, their families, and providers in home and office settings

- Reliable transportation
- Ability to speak multiple languages preferred (i.e., Spanish, Portuguese, Creole, Vietnamese)
