Registered Nurse (RN) / Licensed Practical Nurse (LPN)

Position Summary
The RN/LPN provides high-quality nursing care to residents in accordance with state regulations and facility standards. This role involves assessing resident health needs, administering medications, implementing care plans, and collaborating with the healthcare team to ensure optimal outcomes.
Key Responsibilities
- Deliver direct nursing care to residents in long-term care and assisted living settings.
- Administer medications and treatments as prescribed.
- Monitor residents’ health status and respond promptly to changes.
- Develop and update individualized care plans.
- Supervise and support CNAs and other team members.
- Maintain accurate documentation and comply with all regulatory requirements.
- Communicate effectively with residents, families, and interdisciplinary teams.
- Adhere to infection control and safety protocols.
Qualifications
- RN: Active Indiana RN license; Associate or Bachelor’s degree in Nursing (BSN preferred).
- LPN: Active Indiana LPN license; completion of accredited practical nursing program.
- Experience in long-term care or geriatrics preferred.
- Strong clinical assessment and critical thinking skills.
- Excellent communication and interpersonal abilities.
- Ability to work flexible shifts, including weekends and holidays.
Physical Requirements
- Ability to lift up to 50 lbs and assist residents with mobility.
- Frequent standing, walking, and occasional bending or stooping.
