Show Me Supports, LLC offers Individualized Supported Living, Shared Living (Host and Companion Home), and a variety of Personal Assistant services for individuals with IDD in Southwest Missouri in order to enhance their lives through offering full participation and integration into their community.
Essential Responsibilities/Job Duties
- Staff training and supervision;
- Quality enhancement monitoring;
- Direct plan implementation for individuals as needed;
- Monitoring implementation of outcomes;
- Establishing information collection systems;
- Writing monthly reviews;
- Oversight/coordination of all the person’s programs and services being received;
- Coordinating the development of the ISP (scheduling, facilitation and summary document).
Qualifications:
- 4 year degree or qualifying experience
- 18 years of age or older
- Valid driver's license
- Ability to communicate (verbally and written) with all levels of personnel, internal and external to the company
- Must maintain state regulatory certifications, as required by state/program requirements
