Candidate should hold a Master's degree in Social Work (MSW), Gerontology, or related degree with experience working with the older adult population and/or their families in Hillsborough county. Responsibilities include: assessment, on-going oversight and management of the older adult, care plan development, assistance with placement, advocacy in senior communities and facilities, education and informal counseling.
